Background Check Investigations for ALL VOLUNTEERS

Our Parent Volunteers are an interracial part of Tiverton Middle School's efforts each and everyday. Learn about ways that you can join our volunteers in making Tiverton Middle School a great place for our students to achieve their highest potential. For your convenience, Background Check Investigation (BCI) paperwork is available online at: The Tiverton Police Department (TPD) conducts the BCI on Tuesdays from 9:00 A.M. to 11:00 AM and is located at 20 Industrial Way in Tiverton. BCI paperwork costs $5.00 at the time of service. TPD accepts cash, money order or a check (made payable to Tiverton Police Department). The TPD now requires a letter of intent for all volunteers on formal letterhead of any organization that will be requiring the BCI. Please see any of our administrative staff to obtain a letter from one of our principals. TPD also requires a valid state or federal Identification card that includes your date of birth. Once your BCI is submitted to a member of our central office team, your BCI on file is considered valid through the next four years by the Tiverton School District. A current BCI is required to be on file at our central office, BEFORE volunteering anywhere within the Tiverton School District.

As an additional option, the Rhode Island District Attorney's office conducts BCI's as well. All in-person state and national background checks will ONLY be available at their new Customer Service Center located at 4 Howard Avenue in Cranston (located in the Pastore Center).

 PTO student volunteer form.pdf - Used by students who will be volunteering for the TMS PTO in order to gain volunteers hours that count toward their graduation requirement. Print this form and bring this with you on the time(s) that you will be volunteering. There is a second page that includes a tracking sheet for hours. Both pages must be filled out before submitting to the Volunteer Hour Coordinators.

 Printable Box Tops Collection Sheets  - Tiverton Middle School collects box tops from our students. We appreciate every single one and all box tops can be turned in at the front office within the collection tub m
arked "Box Tops". They do not need to be on a collection sheet. A plastic sealed bag is also acceptable. Thank you for clipping and stay tuned to our quarterly contests that we hold. You could win an amazon gift card! 

All fundraisers must be pre-approved by the building principal and the superintendent prior to advertisement via the District Fundraiser Request Form. Upon completion of the fundraiser or the end of the school year for passive fundraisers, The following fund distribution form is due to the superintendents office within one week of the completion of the approved fundraising efforts. In the event that the fundraiser is cancelled due to weather or lack of participation, a Fund distribution form is still required noting the
event has been cancelled.

Passive Fundraisers 
Passive fundraisers allow all TMS families the opportunity to earn money for Tiverton
Middle School without selling to friends and family. The money earned from these
programs supports efforts of the PTO within the Tiverton Middle School.

Box Tops for Education: Box Tops can be found on a variety of grocery items. Each
Box Top is worth 10 cents. Go to to register as a
supporter for Tiverton Middle School. Also, you can download the app on your smart phone to benefit from your box tops that gives TMS PTC a total of 0.20 per box top!

Stop and Shop A+ School Rewards:  You must register each year. Grab your Stop and Shop card and visit to register your card and designate TMS. If you want to register your card over the phone, please call 1-877-275-2758. This program allows you to
choose two schools if you have children in other schools. Please let all of your loved ones know, friends and family members can participate by designating the PTC as your benefactor when shopping at Stop & Shop. This really helps!  There is no cost to participate and all you need to do is swipe your card each time you shop at the store. TMS ID#10809.

Shoparoo–  Please download Shoparoo to your smart phone if you haven't already. The app is super easy to use; all you need to do is take a picture of your receipts when you go shopping and you earn roo points. Those points turn into cash for our school!

Clements and Lees Register Receipts:
Save your cash register receipts from these
markets and send them in to school. Both stores donate 1% of your shopping total back
to the PTC. The community helps the community! 

Dr. Matthew Forster, Orthodontist: Dr. Forster donates $50 to TMS for each new
orthodontic patient who is a student. His office is located at 1061 Fish Road (in the
business park across from the police station).
